This this is the F.I.D.A. Format for reports. For an example go to Here.

Pleas note, that all reports must go trough SCP Foundation Censorship, so information important to the the report is censored in the Corresponding SCP, Censorship is applied.

[[tab Detainment Report]]

**Date:** MM/DD/YY

**Location Detained:** (Location does not have to match corresponding SCP, Location must be in United States which include: States, Territories, and parts of claimed bodies of water.)

**Recovered Anomaly:** (Give a Very Generalized description of corresponding SCP)

**Agent Casualties:** (A Wounded to Death ratio will be placed here. If no casualties happen put a - for zero. Example: " 3 / - Wounded to Death Ratio")

**Property Damage:** (Will be put in as a monetary value. If no property damage happens, use -)

**Cost of Detainment:** (There will always be a Cost for detainment since the the cost will always consist of Gas for getting to location, Property damage if exists, Casualties if exists, etc. Rounding is okay.)

**Time taken to Detain:** (Time should be realistic compared to location and the danger the corresponding SCP would cause.)

[[/tab]]
[[tab Research Report]]

**Date:** MM/DD/YY

**Anomaly Code:** A-(Threat Level1)-(Month of detainment)-(Day of detainment)-(Year of detainment)-(State number2)

**Physical Description:** (Full description of looks corresponding SCP)

**Properties:** (The anomalous abilities of corresponding SCP)

[[/tab]]
[[tab Test Report(s)]]

[[size 150%]] **Test Number (Threat Level)-(MM)-(DD)-(YY)-(State Number)-(Number of Test)** [[/size]]

**Date:** MM/DD/YY

**Required Supplies:** (All supplies needed for test. Be as specific as possible, down to a number 2 pencil)

**Test Location:** (Choose a location to test anomaly, if test takes place outside of U.S. territory and on foreign territory, a cover-story must be made.

**Test Procedures:** (This is the plan of the Test. If existing test of corresponding SCP exists, rewrite it in the format. Be as Detailed as possible.)

**Test Execution:** (This is what happened during the test. Details are not needed but are welcome.)

**Total time of Test:** (Put a realistic Time for the test.)

**Property Damage:** (Follow the rules back in the detainment report for Property damage.)

**Causalities:** (Follow the rules back in the detainment report for Causalities.)

**Total Cost:** (Follow the rules back in the detainment report for cost. All required equipment should be put into report. Rounding is okay)

**Test Findings:** (Write all of the findings from test. Details are not needed but are welcome.)

(If more test happen, copy the test report but fill out new data.)

[[/tab]]
[[tab Relocation Report]]

**Date:** MM/DD/YY

**Anomaly Code:** A-(Threat Level)-(MM)-(DD)-(YY)-(State Number)

**Properties:** (Reuse Properties from Research Report)

**Selected Relocation Area:** (Find a good place to put to keep the anomaly from threatening U.S. Security. If threat is 3 or 4, Anomaly should be placed to in a military base unless it could cause a Threat to national security.)

**Location Safety Procedures:** (Procedures should make sure that the Anomaly can not ever allow a threat to national security.)
