Special Containment Procedures: SCP-XXXX is to be contained in a 5-Meter by 5-Meter room. The room is to contain a motion detecting device, a single turret, and a video surveillance camera. The footage from this camera is to be watched by an artificial intelligence 24/7 to ensure no instances of SCP-XXXX-01 cross over. There is to be zero visual contact by any personnel and SCP-XXXX-01 in order to prevent harm any specimens that cross over. In the event of a coordinated assault by SCP-XXXX-01, visual contact by security staff is permitted in order to contain any attacking instances of SCP-XXXX-01.
Note by Head Researcher on SCP-XXXX, Richter Moseley: While there is no evidence that any instances of SCP-XXXX-01 intend to assault the Foundation site and break containment, they have likely prepared by now for a similar assault conducted by us, as a result, preparation is necessary to avoid any preemptive strikes that may occur.
Description: SCP-XXXX is a motionless black portal located next to a lake in the ████ province of Russia. It floats exactly a half a meter above the ground and is crossed via direct contact with the portal. SCP-XXXX was first discovered by The Foundation on June 5th, 2017 by field agents sent to track the cause of several disappearances reported by small villages in the region. Upon discovery of the portal, the area was secured by Foundation Staff. The area, labelled Area-███ had been established within a month, with a building built around the anomaly to house it as a precautionary measure. Researchers and security staff were assigned to begin research shortly after, headed by Dr. Richter Moseley.
Addendum XXXX-01
Research conducted on the portal over the course of 2 weeks indicated that there was potential for a drone insertion into the portal, and as a result a single quad-copter drone was programmed to fly into the portal, record footage of the area directly surrounding the other side of the portal, and then promptly return.
Review of recorded footage as documented by Dr. Richter Moseley, lead researcher on SCP-
XXXX: Upon crossing through SCP-XXXX, footage reveals that the area surrounding the exit to the portal is nearly identical to the surroundings of SCP-XXXX on our side of the portal. The area around this end of the portal, hovever, lacks the Foundation post we have established on our side.
Further research will be required in order to discover what else is beyond this portal.
After discovery that the surroundings of the portal's exit were identical to that of Earth, Mobile Task Force Omega-29 "Globetrotters" was assigned to oversee further expeditions into SCP-XXXX. The first expedition took place September 9th, 2017 with the primary objective of seeking out a nearby town in order to discover the differences between Earth and the imperfect copy of Earth.

Other Information: This mission log is a transcript of audio and video taken from recorders of all involved members of MTF Omega-29, as well as the command audio taken from Dr. Moseley's console, it is to be noted that once the crossover had occurred, audio completely cut out from the squad to command without warning, and vice versa, indicating any communication is impossible between the two sides of SCP-XXXX.

Epsilon: Confirmed.
Omega Actual: Alright, you are clear to enter SCP-XXXX, we do not know if communications will work once you enter, as such, follow your objective exactly, if successful we will be able to perform further expeditions.
Epsilon: Understood, entering SCP-XXXX now, mission time starts now.
All members of MTF Omega-29 enter the portal, once everyone appeared on the other side, preparations for the next phase of the operation were started.
Epsilon: Command, do you copy? Repeat, command, do you copy?
Omega Actual: MTF Omega-29 do you copy?
Epsilon: One last time, Command, if you can hear us, please respond.
Omega Actual: Contact has officially been lost with Mobile Task Force Omega-29 at mission time 5 seconds.
Kev: It appears we are on our own from here.
Epsilon: Correct, as such, remember our objective, reach recon point Alpha and then return. Maintain radio silence until arrival to the objective, no contact is to be made with any natives, does everyone understand?
Others Yes Sir.
MTF Omega-29 sets out in the direction of mission objective Alpha, a hill overlooking a village nearby SCP-XXXX on Earth
Sharkbait: Anyone else find it kinda eerie that we're on practically the exact same Earth as our own, just before The Foundation set up an area around here? It's like a mirror or something.
Epsilon: What part of "maintain radio silence" did you not understand Sharkbait? Keep quiet until arrival to the objective, it isn't much further than this.
Sharkbait: Yessir.
Upon the arrival of MTF Omega-29, the town located on Earth was visible below the ridge, exactly as it was on Earth. There are several humanoids walking around in the town.
Kev: What the hell.
Pixie: So we're in a mirror dimension then?
Ranger: Do you think it is possible we could go down and investigate the town, perhaps we've gone back in time by a week.
Epsilon: No, we are to stick to objective and return to SCP-
Epsilon is interrupted by the sound of a screech, twenty meters to Epsilon's left, two humanoids have entered an enraged state. They charge towards the members of MTF Omega-29
Sharkbait: Uh… what the hell?
Pixie: Epsilon, orders?
Epsilon: All units, prepare to engage.
In the time it takes for MTF-Omega 29 to react, a humanoid has already charged and brought Sharkbait to the ground.
Sharkbait: Please dear god, someone help me already!
Kev and Pixie attempt to remove that humanoid from Sharkbait, meanwhile Epsilon and Ranger have restrained one of the humanoids, which continues to rage and fight, prompting Epsilon to hold it down while Ranger delivers a gunshot to the head. By the time the other humanoid is removed from Sharkbait and shot, he is unconcious. Both of Sharkbait's eyes have been scratched out, with the rest of his face covered in scratch marks.
Pixie: Epsilon sir, we need to evacuate immediately, Sharkbait requires immediate medical attention.
Epsilon: Right, Kev, you prepare to carry Sharkbait back to the portal, Pixie and Ranger, you're tasked with carrying the corpses. Let's move quickly, we've already drawn quite a bit of attention and I do not want more of them to come after us.
MTF Omega-29 returned to the portal with no further incident
Epsilon: Alright team, lets get back home quickly, everyone in the portal.
Others Yessir.
MTF Omega-29 enters through the portal
Epsilon: Command, do you copy, we require medical attention for one wounded MTF member. We also brought back some specimens for the eggheads.
Omega Actual presses an intercom button All infirmary staff, a code 45 has just arrived, prepare quarantine measures until exposure to anomaly has been deemed safe.
Omega Actual presses another button Dr. Moseley, prepare for the arrival of 2 anomalous specimens, it appears Omega-29 brought backa few presents.
Post-Mission Notes written by Dr. Moseley: While the injury of Omega-08 was quite unfortunate, the specimens that have arrived will help augment my research and provide us a level of knowledge needed to understand this anomaly. Autopsy will be performed tomorrow to discover the exact nature of the corpses brought back by MTF-O-29.
Final Status: Total Time: 1.5 Hours. Mission was successful with all objectives complete, 1 casualty (Wounded), 2 anomalous specimens recovered.
Addendum XXXX-02
Although unintentional contact had been made with the inhabitants of this other Earth, the results of the autopsy helped further research into the nature of SCP-XXXX. Autopsy revealed that the humanoids that inhabited the other Earth were only superficially similar to humans, and thereby anomalous, being labelled SCP-XXXX-01, nicknamed "Denizens".
Report written by Dr. Moseley two hours after the specimens were recovered from the other side of SCP-XXXX: it appears that these specimens, while human-like in nature, are more distant than initially believed upon footage review.
The initial physical check denoted that these specimens, hereby named SCP-XXXX-01, are not in fact human, but rather some distant evolution. Instances of SCP-XXXX-01 appear to have a total of 6 fingers and toes, with their nails seemingly coming to a natural point unlike human nails. Upon dissection of one of the two specimens received, it was revealed that they seemingly have their organs not only mirrored, but have two extra organs of unknown use in the abdominal cavity. The brain contains no notable difference from humanity, neither are differences noted in their facial features.
It is to be noted that shortly after the autopsy, 3 Specimens of SPC-XXXX-01 entered the facility through SCP-XXXX. Upon being sighted by a security officer on surveillance duty, it appears that the security officer entered an enraged state upon sighting them on camera and promptly attempted to open the security door to the containment chamber. The audio log below contains the events that occurred after additional security arrived in an attempt to ensure containment of SCP-XXXX.

Security Staff Member-02: The hell was that?
Security Staff Member-03: No idea, briefing didn't mention there being any real issues with containing this skip.
Security Staff Member-02: Exactly, let's move to ensure that SCP-XXXX doesn't breach containment.
Security Staff Member-04: How is a portal going to breach containment. (Note: Security Staff had not yet been properly briefed on the lifeforms located on the other side of the portal to this point.)
Security Staff Member-02: The portal is an unexplained anomaly, maybe [REDACTED] Security Staff Member-01 triggered something on it. [REDACTED] Security Staff Member-03 and [REDACTED] (Security Staff Member-04) you both come with me to investigate the situation. [REDACTED] (Security Staff Member-05) You radio in and alert the Chief Security Officer that there may be a potential breach by SCP-XXXX, hurry.
Security Staff Member-05: Got it! Footsteps can be heard as Security Staff Member-05 exits the room to radio to the Security Chief.
Footsteps can be heard as the remaining security staff make their way towards the containment chamber of SCP-XXXX, footsteps slow to a stop as repeated banging is heard on a door.
Security Staff Member-04: Okay seriously, what the hell?
Security Staff Member-02: [REDACTED] (Security Staff Member-01), what in gods name are you doing?
Security Staff Member-03: He must really hate that door.
Security Staff Member-02: Cut the crap [REDACTED] (Security Staff Member-03) and detain him. I'm going to open the containment door to ensure the portal is still secure.
Footsteps can be heard from multiple sources as Security Staff Member-03 attempts to detain the Surveillance Officer and Security Staff Member-02 approaches the door control.
Security Staff Member-04: Wait, what if it's the skip that's causing him to go crazy, opening it will just expose us to-
Security Staff Member-04 is cut off by the sound of the containment door opening, Security Staff Members 03 and 04 rapidly fall into the same rage as 01 and large amounts of footsteps can be heard as they collectively run into the containment chamber.
Security Staff Member-02: Damnit, what just happened?
Screaming can be heard as a fight breaks out between the security staff in the room and the instances of SCP-XXXX-01. The sound of a radio activates.
Security Chief: [REDACTED] //(Security Staff Member-05) just radioed in and alerted me to a possible breach, reinforcements are en route as we speak, can you give confirmation to your team's condition and current position?
Security Staff Member-02: Currently three members of my team have gone insane due to an unknown anomaly related to SCP-XXXX, it looks like whatever caused this is related to sight, currently the team is fighting whatever's in there. Wait, the fighting's died down.
Security Chief: Do not attempt to investigate, wait until reinforcement teams arrive.
Security Staff Member-02: Understood. Wait, is that. I think one of the team survived, he's about to walk out no-
Roars can be heard as Security Staff Member-02 falls into an enraged state, footsteps are heard, followed by a struggle and a head being slammed on tile, shortly after, Security Staff Member-02 appears to calm, with breathing able to be heard returning to normal.
Security Staff-02 What? A radio can be heard activating Chief, we're going to need a cleanup crew here.
End Log
Review of Surveillance footage inside the room and hallways revealed that upon sight of the three instances of SCP-XXXX-01, all security staff who had been visually exposed to it were instantly thrown into a violent rage, only ending after the death of all known instances of SCP-XXXX-01 in the area. Footage revealed that at least one of the three instances had escaped the room while the other two had remained to fight, killing Security Staff Member-01 in the process, before being killed by Security Staff Members 02 and 03.
Much like the corpses brought from the MTF expedition, dead instances of SCP-XXXX-01 have no notable effect on the actions of humans in the area.
Addendum XXXX-03
Upon further review, a theory has been put forth by Dr. Richter Moseley attempting to explain the anomaly. The theory describes SCP-XXXX as a portal that leads to another dimension, as opposed to a theorized mirror-world or other planet. This dimension simply contains an alternate Earth in which humanity evolved slightly differently than in our world.

Special Containment Procedures: A single instance of SCP-XXXX is to be stored at every Foundation Site inside of a waterproof storage locker, use of the item for research or by infirmary staff is only permitted with the approval of a Site-Director or higher. Prior to use, an instance of SCP-XXXX must be copied and the original copy returned to its locker. In the event that a copy of SCP-XXXX is destroyed, a new instance of SCP-XXXX will be sent to the site with 05 approval.
Description: SCP-XXXX is a stack of basic medical records, which, when a name is written into the "name" column, a description of the person's medical history will appear in writing onto the paper. The medical history includes current health status, allergies, any current injuries sustained, oral intake in the last 24 hours, prior medical history (I.e. operations, illnesses, list of medications taken, illicit substances, etc.), and vital signs at that time. The records seemingly distinguish who's record is taken based on whoever is being thought of by the writer at that moment. If the named of a deceased subject is placed into the record, a description of the time, date, and cause of death of that subject will appear, indicating some form of omniscience by the records.
Addendum XXXX-01: SCP-XXXX had first been discovered on 3/6/1998 after the arrest of a wealthy M.D. who had murdered his wife, claiming she had had sexual relations with his brother and gotten pregnant while he was away on a convention trip, despite the wife not yet having shown outwardly visible signs of pregnancy. An undercover Foundation Agent acting as a detective in the local police force was part of the murder investigation and discovered an instance of SCP-XXXX in the M.D.'s desk. It appeared that the doctor, knowing of the anomalous properties of SCP-XXXX, had used it with suspicion of her cheating, with the records confirming his suspicions. There were a grand total of ███ instances of SCP-XXXX collected from the office, differentiated by their vastly simplified charting as compared to typical medical records used by the M.D.'s practice.
Initial testing revealed the anomalous properties of SCP-XXXX, being a perfect record of any subject's health. Further testing indicated that photocopied instances of SCP-XXXX carry the same properties, indicating an indefinite amount of copies of SCP-XXXX can be made and used.
Summary: A town appearing on a single anomalous map, using the map to navigate will lead to this town, otherwise, even if one follows the exact directions without the map, they will not end up in the town. The town is literally made of paper, containing identical paper-people.
