Addendums:

Instance #1: The discovery of SCP-6116;
SCP-6116’s name was found on a wall in ████████ ██. Local Police remove graffiti.

Instance #2: First sighting of the name in the foundation;
SCP-6116’s name was found in a document of SCP-████ along with a note saying: “To Site Director ██████████

It was his fault, and he walked away and nobody knew, the dirty bastard killed an innocent puppy. Was it fair punishment? I would watch my back if I was Mr ██████,

From - Doctor Heinsen”
The person mentioned in the letter was interviewed and was found dead 3 weeks later. The connection to the note and the death is still under question.

Instance #3: SCP-6116 is added to the foundation database.
After some data mining, researchers found Dr. Heinsen was not a real doctor working at that site. There was also no evidence a doctor with the last name Heinsen was ever assigned to SCP-████. The staff who worked on the document have no recollection of adding the name to the document or seeing anyone else adding the name and note. SCP-6116 was added to the foundation database as a potential anomaly.

Instance #4: SCP-6116 is declared an SCP;
After five months of the instance of SCP-6116’s name in an SCP document, it happened again to a document of SCP-████(note shown below)

“To Site Director ████ ███████,

I would like to inform you about this one, this ‘thing’ isn’t what you think it is and you should terminate it as soon as possible. In approximately one week, it will breach containment and cause mass site havoc, it is not some cute thing, it is a murderer, and if you don’t terminate it now… well, God have mercy on your soul.

From - Doctor Heinsen”

No researcher that day had a name of Doctor Hensein at that site, the closest Doctor Heinsen was at a site in Mexico at the time, one week later, SCP-████ escaped containment and created several instances of itself that wound up killing nearly half the site.

Instance #5: SCP-6116’s is found carved into Agent ███ ████████,
When the Agent woke up the next morning, he said he was dreaming about being with his (deceased) Mother, he described the dream as scary and remembers holding her hand while he was getting some kind of injection. He said he felt no pain through the dream and when he woke up, he only felt pain when they noticed the name. The cameras were checked later to see if the individual who did this could be caught, but the cameras malfunctioned and the only footage left from it was from the morning, and when the Agent climbed into bed. Further investigation is underway.
