Special Containment Procedures:
Entity is to be stored in a wooden box with a glass lid at all times except when needed for research or interviewing purposes. Exceptions can only be made by Level four (4) personnel or higher.
The wooden box is to be stored inside a pressurized chamber, measuring exactly 140 bar, with the following dimensions: one (1) m x one (1) m x one (1) m and made out of reinforced concrete. The chamber must be secured to the ground below with four (4) stainless steel rods, measuring at least fourty (40) cm in diameter.
Said chamber is currently located at site 18, below 100 meters of the surface
The door to the chamber is to be kept tight shut, tripple locked and under 24/7 video- and audio surveillance and recording.
The box has to be stored laying down, the glass lid always pointing towards the ceiling to the video- und audio surveilance and recording device in an effort of uninterrupted surveillance and recording.
SCP-XXXX:
THE TEXT YOU WANT SHOWN UNDERNEATH YOUR IMAGE
The Entity SCP-XXXX is a skeleton metal key, made out of iron and a silver polishing, with two key bits, and an engraving on one side. The engraving reads █████. No manifacturing company or wear and tear can be found on the key.
The Entity is reported to being able to shapeshift in a human-like creature, resembling some descriptions of "The Grim Reaper" or, more commenly known as, "Death". When shapeshifted into this human-like creature (SCP-XXXX-1), it measures two (2) meters tall, weighing about 100 kilogramms and clothed in a black robe with a sickle in his left hand. The robe seems to be made out of cotton, no tests have been conducted as of writing this report. The robe covers the whole appearence of the Entity, besides the left hand which is completely skeletal. It seems like the robe is somehow attached to the Entity.
When shapeshifted to SCP-XXXX-1 the Entity is capable of speaking. The voice(s) seem(s) to be disembodied and made out of different voices (much like layers). The Entity is not hostile towards the Foundations personnel and is well spoken.
The Entity (SCP-XXXX) was brought to the Foundations attention after it had appeared in numerous dead bodies found around ████████████, in the county of ████████, by the ████████ Police Department. The investigating homicide detective, Mr. Elias ████████, found the Entity around 20██. After it went missing everytime it had been recovered from a dead body the investigating detective contacted the Foundation personnel. It was brought in in the wooden box it is in now, claimed by the detective that it was the only way to keep it from escaping. Tests about this are currently pending.
The whole process of the shapeshifting into SCP-XXXX-1, and what it's purpose or agenda is, is not fully understood by the Foundation personnel as of right now. Furhter research and tests are needed and given clearence by Dr. ████████.
Addendum XXXX.1: Incident Report 13/5/18
As of May 13th 2018 two members of the Foundation's personnel are required to monitor the Entity through the video- and audio surveillance system 24/7, in four (4) shifts of six (6) hours and each personnel shall not exceed more than 24 hours a week monitoring the Entity.
After Containment Breach on May 13 2018 at 0900 hours the forementioned instructions are to be put into place immediatley.
The Entity escaped from it's wooden box. The box started violently shaking at around 0850. The bulletproof glas at the top shattered and the key started floating in the containment room, even though the pressure resumed at 140 bar. At 0855 the security cameras went out and thrity seconds later came back on, revealing a figure resembling a grim reaper (SCP-XXXX-1). The containment room had changed and now measured approx. four (4) x four (4) x six (6) meters.
On site personnel immediatly contacted TAU-Unit 9. After releasing the pressure the Unit entered the room only to find very distorted picture where the wooden box once had been.

Even though TAU-Unit 9 couldn't see SCP-XXXX-1, reports of the monitoring personnel clearly stated that it had been present in the room. TAU-Unit 9 and on site monitoring personnel tried taking pictures but ended up only with the distorted picture shown above. Viewing the tapes of the surveillance cameras revealed nothing as they only showed the wooden box laying in its place. It seems that Entity SCP-XXXX-1 can only be viewed by direct means through the human eye.
TAU-Unit 9 was instructed by Dr. ██████ to leave the room at 0900 and special containment procedures were resumed.
Since this instance took place the Entity SCP-XXXX-1 hasn't changed or left the room it has been contained it.
Addendum XXXX.2: Interview with SCP-XXXX-1 after Incident Report 13/5/18

Interviewer: Seeker

Seeker: Hi SCP-XXXX-1. My name is Seeker. I am here to conduct an interwiev with you after the incident on May 13th 2018.
SCP-XXXX-1: Greetings Seeker, or shall I call you Mr. █████ █████?
Seeker: Seeker will do just fine. Why did you break out of your wooden box and shapeshifted into SCP-XXXX-1?
SCP-XXXX-1: I am not bound to any object. I choose my appearence as I wish. It was time for me to talk to someone of your Foundation.
Seeker: Well I see. What do you want to tell us?
SCP-XXXX-1: Your Foundation currently contains SCP-049, also known as "The Pleague Doctor". He must be terminated.
Seeker: What is your connection to SCP-049 and why does he need to be terminated, as you say?
SCP-XXXX-1: Our connection, as you call it, is nothing of your concern. He is very powerful and needs to be stopped in what he is doing right now.
Seeker: Okay… I see. Would you care to elaborate on that statement?
SCP-XXXX-1: Most certainly I will. He is messing with the order of the universe. I tried to stop him myself ever since he appeared but I only could keep him at bay. Ever since he was contained by your Foundation he grew stronger and is on it's way to bring the whole universe to a collapse.
Seeker: When you say he is messing with the order of the universe, what do you mean by that? What we geathered is he is trying to find a "Cure" for this "Pestillence" he is talking about.
SCP-XXXX-1: I know what he calls it. It's irrelevant what he is trying to find. Your Foundation needs to make sure his "studies" don't progress any further. It will lead to a collapse of the whole universe and everyone will perish within it.
Seeker: Okay. We still don't understand what he is trying to find and what he is trying to cure exactly. Do you know what…
Interviewer Seeker get's interrupted by SCP-XXXX-1 as loud thunder can be heared in the room, making the whole room vibrate.
SCP-XXXX-1: I AM NOT HERE TO DISCUSS THIS MATTER. MAKE HIM STOP!
The thunder stops and SCP-XXXX-1 vanished, leaving only the skeleton key behind he shapeshifted from.

Closing Statement:
After the interview had been concluded SCP-XXXX had been put into another wooden box with a more advanced bulletproof glas and currently remains under the special containment procedures stated above, including Addendum XXXX.1.
Interviewer Seeker was visible shaken and after writing down this log wished to never talk to SCP-XXXX-1 again as he feared for his life. He underwent psychological evaluation and was found unfit to work for the Foundation ever again. Interviewer Seeker was given Class A amnestics and is released from any duty within the Foundation.
An interview with SCP-049 was requested by Foundation personnel and had been granted by Dr. ██████.
Review of this log by Dr. █████ concludes that the Entity is Ontokinetic.

Addendum XXXX.3:
Since the containment breach on May 13th 2018 and the following interview the Entity SCP-XXXX has not shapeshifted into SCP-XXXX-1 and remains in his containment chamber. As of right now the interview with SCP-049 is outstanding. Foundation personnel has no knowledge on how to trigger the shapeshifting mechanism or how to contact either one Entity.
Foundation personnel has been made aware of the suicide of Interviewer "Seeker" on May 31st 2018 at approx. midnight. He left a suicide note but nothing was written on it. It only showed the distorted picture already known to the Foundation. It is currently unknown if "Seeker" drew the picture himself or if he somehow contained it from within the Foundation.
An autopsy will be performed by Foundation personnel as to determine if the given Class A amnestics did not work as intended or if memories of the interview somehow got restored in his brain.
